Be sure to have the printed information available, just in case you run into this. A set in your glove compartment is always a good idea. I went to your site and found the free downloads section at the bottom. The list of available forms is GREAT and the book you've written looks terrific as well! I would encourage people to also print out the information as well as copy it to a flash drive.. Not all medical sites are as up-to-date with technology as we would like, plus flash drives can be lost/damaged.
