		<description>This is such a chronic issue-  The hospitals have not adapted to the changing face of  what old age means, and seem unwilling to learn how to handle these cases effectively- and keep elderly from returning time after time.  As a case manager in a rural area  it becomes critical to help aging adutl reduce hospital time-  families are not able to drive two hours each day to see their loved ones
